I would like to post a digital version of my informed consent form (saved as a .jpg file) as the first page of my web-based survey. Can anyone share a script to post a full-screen picture? I would only need a "next" button to appear (no items for participants to complete). 

You can set up an appropriately sized <image> on a <surveypage> to do that.
