I have been searching for a reliable consent form template. How do I create a consent form without attaching an image or HTML page? I tried a surveypage but had numerous problems I couldn't rectify, and I even tried a custom likert model but I couldn't complete the script. Any suggestions? I looked for a template on Millisecond, but the scripts were DemoBatch only, or the template couldn't be broken down.
Thanks for any input - total novice here.

Depends on what you mean by "consent form" and how you plan to use it.
#1: If you are aiming for web use and want your consent form displayed *before* participant's ever start a script, you need to use a HTML file. Any word processor should be able to "save as" HTML, so type in whatever you need, save the file as HTML, upload it along with your other files and specify that file as your consent page via your web experiment's settings.
#2: If you want to ask for consent in your script, any regular <trial> or <surveypage> can serve that purpose. There is no need for images or HTML in this case. For a simple example using a basic <surveypage>, see e.g. the files attached here: https://www.millisecond.com/forums/FindPost19224.aspx
