It's not possible using <likert>. You can of course create a custom likert-type scale using regular <trial>, <text> and/or <picture> elements. In that latter case, you can add a <button> that is not part of the scale as an addition response option.

You specify the button stimulus as /validresponse in the <trial> element, just like you specify the text and/or picture stimuli that make up the scale there. That is all, you don't need to check mouse button status or position.
