One more thing.... How do you make a space between a caption and the labels when using a slider question?

Survey questions will always be left-aligned. You can, however, use their respective /position attributes to move them towards the center of the screen:

<radiobuttons myradiobuttons>
...
/ position = (40%, 10%)
...
</radiobuttons>

Thank you! I have moved the questions with the /position attribute. However, the /caption at the top of the <surveypage is still left aligned and I still can't figure out how to centre this, as there is no /position attribute available for this element!!
Any suggestions?

Don't use the <surveypage>'s /caption attribute. Instead set up a <caption> element and use its /position attribute.

You would include all of that in the /labels attribute:

<slider myslider>
/ labels = ("A~n1","~n2","B~n3","~n4","C~n5")
/ range = (1,5)
</slider>
