It's impossible to tell what went wrong based on only that screenshot. Which script was used? Which operating system? Which browser? Which Inquisit Web version? What other software was running on that system at the time (the error *could* indicate another application interfering with Inquisit)? Were the machine's display drivers, etc. up to date? Have you or others been able to reproduce the issue? Has the colleague in question been able to consistently reproduce it?
