Improperly recorded survey data files

Hi everyone, 
The data files for a survey I created seem to be improperly recording. Specifically, the column labels align with the survey responses correctly up to a certain point, after which the responses become shuffled (seemingly randomly). 
I was hoping to solve this problem by using the data element to custom record only the information we need; latency, for example, is not important for our purposes. However, every piece of information I find about the data element seems to be specific to performance tasks.
Does anyone have information about how to customize survey data recording? Has anyone else had an experience with improperly organized data files?
Any help would be greatly appreciated!

Data output for <survey> elements cannot be customized. If you want to customize data output, you must use <block> elements to run your <surveypage>s.

As to why your survey data files may be improperly formatted: The cause is impossible to determine without the code for the actual script. Do take note though that your Inquisit version appears outdated (4.0.5.0). I would strongly recommend installing the latest release.
